Sensitization to Cor a 9 and Cor a 14 is highly specific for a severe hazelnut allergy in Dutch children and adults

چکیده
Results IgE to hazelnut extract was significantly higher in children with severe than with no or mild symptoms. Sensitization to rCor a 1.04 was common among both children and adults, while sensitization to rCor a 8 was rare. In 13% of children and 49% of adults with a severe hazelnut allergy, only sensitization to rCor a 1.04 was observed and not to other water-soluble allergens. Sensitization to nCor a 9 and/or rCor a 14 was strongly associated with a severe hazelnut allergy. Using adapted cutoff levels, a diagnostic discrimination between severity groups was obtained. IgE to either nCor a 9 ≥1 kUA/L or rCor a 14 ≥5 kUA/L (children) and IgE to either nCor a 9 ≥1 kUA/L or rCor a 14 ≥1 kUA/L (adults) had a specificity of >90% and accounted for 83% of children and 44% of adults with a severe hazelnut allergy.

Sensitization to cor a 9 and cor a 14 is highly specific for a hazelnut allergy with objective symptoms in dutch children and adults.
BACKGROUND Component-resolved diagnosis has been shown to improve the diagnosis of food allergy. OBJECTIVE We sought to evaluate whether component-resolved diagnosis might help to identify patients at risk of objective allergic reactions to hazelnut. METHOD A total of 161 hazelnut-sensitized patients were included: 40 children and 15 adults with objective symptoms on double-blind, placebo-c...
متن کاملAge-dependent sensitization to the 7S-vicilin-like protein Cor a 11 from hazelnut (Corylus avellana) in a birch-endemic region.
BACKGROUND Hazelnut (Corylus avellana) allergy exhibits age and geographically distinct sensitization patterns that have not yet been fully resolved. OBJECTIVE To study sensitization to Cor a 11 in different age groups of hazelnut-allergic patients and infants with atopic dermatitis (AD) sensitized to hazelnut in a birch-endemic region. METHODS Sera from 80 hazelnut-allergic patients, 33 in...
متن کاملO11 - Cor a 14: the missing link in the molecular diagnosis of hazelnut allergy?
Results Thirty-seven patients suffered from a systemic reaction and 38 patients reported an oral allergy syndrome (OAS) after eating hazelnut. In the population with systemic reactions, sensitization to Cor a 14 was seen in 19/20 preschoolchildren (median age (range) 2.6 years (1.0 – 5.4)), 8/10 schoolchildren (10.2 years (8.0 – 13.8 )) and 2/7 adults (28 years (18 – 33)) whereas sensitization ...
متن کاملSensitisation to Cor a 14 and Cor a 9 is a risk marker for severe hazelnut allergy in children
Background Hazelnut-allergic children express variable clinical phenotypes. Tests for the diagnosis of hazelnut allergy are usually conducted on children presenting clinical symptoms following hazelnut ingestion, but also as screening tests after the discovery of a food allergy, mainly to tree nut or peanut. It is important to determine if molecular allergens could be markers of severity in haz...
متن کاملPredictive values of component-specific IgE for the outcome of peanut and hazelnut food challenges in children.
BACKGROUND Oral challenges are the gold standard in food allergy diagnostic, but time-consuming. Aim of the study was to investigate the role of peanut- and hazelnut-component-specific IgE in the diagnostics of peanut and hazelnut allergy and to identify cutoff levels to make some challenges superfluous. METHODS In a prospective and multicenter study, children with suspected peanut or hazelnu...
